The Home Visit Program

First responders see seniors in crisis every day — frequent falls, unsafe living conditions, isolation, and inability to care for themselves. These calls don't always need an ER visit. They need a follow-up care plan.

Our Home Visit Program partners with fire departments and community paramedicine teams to create a warm handoff. When you encounter a senior who needs support beyond what emergency services can provide, submit a referral through our secure form. We follow up within 1 hour.

How it works:

• First responder encounters a senior in need during a call
• Submit a referral through our HIPAA-compliant form — takes 2 minutes
• CCA contacts the senior and family within 1 hour
• We assess needs, research options, and coordinate the right level of care
• Follow-up report sent to the referring department

The program is free for departments and for seniors. We also provide quick reference cards your crews can carry on the rig.
